Act of April 19, 1908 Amended Act September 8, 1916 DECLARATION FOR WIDOWS PENSION.

On this _21st_ day of _November_ , A.D. one thousand nine hundred and _sixteen_ personally appeared before me, a _Notary Public_ within and for the county and State aforesaid, _Helen D. Everingham_, aged _sixtynine_ years, a resident of _Iowa_, who, being duly sworn according to law, makes the following declaration in order to obtain pension under the provisions of the ACT OF CONGRESS APPROVED APRIL 19, 1908 _amended by Act of September 8, 1916_ That she is the widow of _Barsnett D. Everingham_, who was enrolled under the name _Barsnett D. Everingham_ at _Dubuque, Iowa_, on the _eighth_ day of _August_, 18_62_ as a _first sergeant_ in _Company I, 38 Regiment Iowa Vol. Inft._, and honorably discharged _August 15th_ 18_65_ having served ninety days or more during the late civil war. That he also served _Co. F. 34 Reg. Iowa Vol Inft._That she was married under the name of _Helen D. Hunt_ to said soldier at _New Oregon, Howard Co. Iowa_, on the _first_ day of _January_, 18_73_, by _Edward Southworth, Pastor Congregational Church_ that there was no legal barrier to the marriage; that she had _not_ been previously married; that the soldier had _X_ been previously married. _September 12, 1871, Former wife died at New Oregon, Iowa_That the said soldier died _November 15th_, 19_16_, at _Cresco, Iowa_ That she has _Not_ before applied for pensionThat her post-office address is _521 -- 3rd St. West, Cresco_, County of _Howard_, State of _Iowa_

Everingham Ottawa, Kansas_, and _Lottie A Kellow of Cresco, Iowa_, persons whom I certify to be respectable and entitled to credit, and who, being by me duly sworn say they were present and saw _Helen D. Everingham_, the claimant, sign her name to the foregoing declaration; that they have every reason to believe, from the appearance of said claimant and their acquaintance with her of ______ years and _______ years, respectively, that she is the identical person she represents herself to be, and that they have no interest in the prosecution of this claim.Stamped: "Declaration accepted as a claim under amended Act Sept 8,1916." & Signed by "Frank & Lottie"
